What support is available when transitions and life changes occur? This webinar will discuss some of the common reasons transitions are needed and provide resource information so that seniors and family members can be empowered to make plans that meet the needs and honor individual wishes for care while being well supported.

Meet Our Panelists

Dr. Tiffany Reed completed her undergraduate and graduate studies back home in Pennsylvania before moving here to North Carolina to pursue her clinician-educator fellowship at Duke University. Since then, she has practiced person-centered geriatric care in Greensboro with Cone Health for over 10 years. Dr. Reed served as a Certified Medical Director of Well-Spring Retirement Community as well as several local skilled nursing facilities during that time. She has recently joined AuthoraCare Collective as Medical Director of Chronic Disease Management and Palliative Care.


Ursula Robinson is the Executive Director of PACE of Guilford and Rockingham Counties, dba PACE of the Triad, located in Greensboro, NC. PACE is an acronym which stands for Program of All-inclusive care for the Elderly. PACE is an integrated, coordinated, person centered, provider led, capitated and fully risk bearing model of care. Driven by objective of maintaining the independence of program participants in their homes and communities for as long as possible; PACE programs are the lifelines that enable frail older Americans to live at home. Ursula served on the planning committee to evaluate the feasibility of the PACE model in Greensboro and was hired as the first Executive Director in 2010. Under Ursula’s leadership, PACE of the Triad is the third largest PACE program in NC and the first PACE program in North Carolina to provide an Alternative Care Setting to serve rural participants.


Lynley SanGeorge is a Licensed Clinical Social Worker. She has worked at AuthoraCare Collective (ACC) as a Medical Social Worker in long-term care for three years. Lynley has started in a new role this past year as the Transitions and Life Changes Counselor at Well-Spring Life Planned Community through a partnership with ACC. In this new role, Lynley has worked with her team to help build the TLC program from the ground up. Lynley uses a Person-Centered therapy approach to guide her practice, which focuses on creating a space where residents can feel safe to openly share their thoughts and feelings. She assists residents to give voice to their concerns and challenges, while collaboratively setting goals using strengths and skills they already possess.


Meet Our Moderator

Risa Hanau, LCSW is the Clinical and Community Educator at AuthoraCare Collective. Risa is a seasoned presenter and teacher in the area of end-of-life and ethics. She is a member of the Cone Health Ethics Committee. She is a Licensed Clinical Social Worker by training and holds a Master of Social Work from The University of Pennsylvania.
