Getting Hospice Care

AuthoraCare Collective serves patients wherever they call “home”—whether it is a private residence, long-term care community, or one of our own inpatient hospice facilities: Beacon Place in Greensboro and The Hospice Home in Burlington.

Hospice in Your Home

Most hospice care is provided in the comfort of one’s own home. Your AuthoraCare team will schedule regular visits to your home to assess, educate and comfort the patient and family.

The frequency of team visits is adjusted based on the needs of the patient, family and caregivers. For example, the nurse typically visits one-two times per week, and the team social worker and chaplain typically visit one-two times per month. If a certified nursing assistant is assigned for personal care, these visits are also intermittent, usually starting at two-three times per week.

Hospice in Long-Term Care Communities

AuthoraCare also serves patients who are currently residing in skilled nursing, memory care, assisted-living, independent-living and group home communities. Our team works closely with facility staff members to serve residents with a life-limiting illness. The frequency of visits is based on the needs of the patient. We are not here to take over care, but to offer expert guidance to the patient, family and facility staff.

Hospice at Our Inpatient Facilities

In addition to providing hospice care in personal residences and long-term care facilities, AuthoraCare also offers more intensive, around-the-clock hospice care in our two inpatient hospice facilities: The Hospice Home in Burlington and Beacon Place in Greensboro.

Care at our inpatient facilities is short-term and focused on symptom management for patients whose care needs are more intensive than can be provided at home.

Admission Criteria for our Inpatient Facilities

Admission to our inpatient facilities requires a doctor’s order. Typically, patients are adults, but our rooms and beds can be easily modified to serve Kids Path patients between the ages of birth to 18.

“The nurse’s notice of signs my husband was leaving us allowed me to hold him, talk to him, pray and say my goodbyes before he was gone. For that I’m eternally grateful. My husband was treated with respect at all times as was our entire family.”
