Palliative Care in Your Home

Most palliative care is provided in the comfort of one’s own home. Your AuthoraCare team will schedule regular visits to the patient’s home. They will coordinate a treatment plan with the patient’s regular doctors to ensure that all care is aligning with the patient’s and family’s health care wishes. 

Palliative Care in Long-Term Care Communities

AuthoraCare also serves patients who are currently residing in skilled nursing, memory care, assisted-living, independent-living and group home communities. Our team works closely with facility staff members to serve residents with a life-limiting illnesses. The frequency of visits is based on the needs of the patient.
